Abortion (Pregnancy Termination) is a surgical procedure that must be performed by an experienced Obstetrician & Gynecologist. Today, the most preferred method is Vacuum Aspiration, which can be performed under local anesthesia or sedation (general anesthesia).

What is Abortion?

Abortion is the name given to the process of cleaning the inside of the uterus to terminate the pregnancy in cases of unwanted pregnancies. This procedure is medically called Dilatation and Curettage (D&C) or Vacuum Aspiration.

  • Termination of Pregnancy: The most frequent reason for application in Ankara.

  • Revision Curettage: Cleaning the inside of the uterus after a miscarriage (incomplete abortion).

  • Probe Curettage: A surgical procedure performed to take a biopsy from the uterus in cases of irregular menstrual bleeding (Diagnostic).

Before terminating an unwanted pregnancy, an ultrasound examination is performed to check whether there is a gestational sac and, if so, to measure the week. The location of the sac is also checked. When the sac becomes visible on the ultrasound (usually from the 4th-5th week of pregnancy), the abortion procedure can be performed.

How is Abortion Performed? (Vacuum Method)

The healthiest and safest method used for pregnancy termination in our country is Vacuum Aspiration. The risks of the procedure performed with the vacuum method are much less compared to old-style methods (metal curettes).

In legal abortion procedures performed by a specialist gynecologist, the procedure takes 5-10 minutes if there are no technical problems. This reduces the patient’s infection risk and makes the procedure psychologically easier to tolerate.

Anesthesia Options:

  • Local Anesthesia: Numbing only the cervix. The patient is awake. Preferred by patients who have had a normal birth before or are afraid of general anesthesia. You can return to normal life immediately.

  • Sedation (General Anesthesia): Short-term sleep. Preferred for a more comfortable procedure in patients who have never given birth or have a low pain threshold. It requires 5 hours of fasting (no food or water) before the procedure. After the procedure, it is not recommended to drive or drink alcohol for a few hours.

When Can Abortion Be Done? (Legal Time Limit)

The legal limit determined by the Ministry of Health for abortion in Turkey is 10 weeks.

  • Over 10 Weeks: In pregnancies over 10 weeks, this procedure can only be performed with a medical board decision if there is a situation threatening the life of the fetus or the mother.

Calculating Pregnancy Week: The most important confusion experienced in determining the abortion week in Ankara is that patients calculate the pregnancy week from the date of intercourse. However, the pregnancy week is calculated from the first day of the last menstrual period (LMP). Ultrasound measurements are also compatible with this date. Therefore, a pregnancy that the patient thinks is 8 weeks old based on the date of fertilization may have already passed the legal limit of 10 weeks.

Can a Virgin Get Pregnant? (Abortion for Virgins)

Unfortunately, yes. Even with friction (outcourse) or ejaculation near the vaginal entrance without penetration, sperm can move into the vagina and cause pregnancy without the hymen being broken.

Can a virgin have an abortion? Yes. When the abortion procedure is performed with the vacuum method, thin plastic cannulas are used. In the first weeks of pregnancy, smaller instruments are used. However, a speculum (a medical tool) must be inserted to see the cervix, which may damage the hymen. The priority during abortion is to ensure no tissue remains inside. Therefore, damage to the hymen (loss of virginity) is a possibility during abortion.

Is Abortion Legal in Turkey?

Yes.

  • Over 18 & Single: Can have the procedure done with her own consent only.

  • Married: Requires the consent and signature of both herself and her spouse.

  • Under 18: Requires parental consent.

Is There an Abortion Pill? (Medical Abortion)

In our country, the use of drugs (such as Misoprostol/RU-486) for abortion purposes outside of hospital control is prohibited. Using these drugs without doctor supervision can lead to complications due to excessive bleeding or incomplete abortion. Therefore, the safest method is Vacuum Aspiration.

Risks of Abortion

When performed by an expert gynecologist under sterile conditions, the risks of the vacuum method are negligible. However, rare complications may include:

  • Infection: Can occur if hygiene rules are not followed post-op. Using antibiotics prescribed by your doctor reduces this risk.

  • Incomplete Abortion (Rest Placenta): If pieces of tissue remain inside, bleeding continues. It may require re-cleaning.

  • Uterine Perforation: A rare risk where the uterus is punctured. It is less common with plastic vacuum cannulas compared to metal curettes.

  • Asherman’s Syndrome: Adhesions inside the uterus. The risk is minimized with the vacuum method as there is no harsh scraping.

Post-Abortion Care

  • Sex: Sexual intercourse is not recommended for 1 week (until bleeding stops and healing is complete) to avoid infection.

  • Hygiene: Do not enter the pool or sea for 1 week. Take standing showers instead of baths. Use pads instead of tampons.

  • Bleeding: Bleeding similar to menstrual bleeding or spotting may occur for a while. Sometimes there may be no bleeding at all.

  • Pain: Mild groin pain is normal; painkillers can be used.

  • Check-up: You should go for a control examination 1 week after the procedure.

Frequently Asked Questions (FAQ)

Will it affect future pregnancies? One of the most frequently asked questions. When performed with the vacuum method under sterile conditions, abortion does not cause infertility. You can get pregnant again as early as 2 weeks after the procedure (ovulation resumes quickly). Therefore, start contraception immediately.

Can a doctor tell if I had an abortion? After the uterus has healed (usually after the first period), it is not possible for another doctor to understand that you had an abortion in a routine examination. Also, if the procedure is done in a private clinic (non-SGK), it does not appear in the general public health records system, ensuring your privacy.

Is my privacy protected? Patient information cannot be shared with anyone other than the patient herself (except for legal obligations). Therefore, patients do not need to worry about confidentiality.

When is the Anti-D (Rh) injection given? If the patient is Rh Negative (-) and the partner is Rh Positive (+), there is blood incompatibility. An Anti-D immunoglobulin injection must be administered within 72 hours after the abortion to protect future pregnancies.
